Membership Form: The 2012 / 2013 Okte PTA Membership Form.

Event Funds: At the end of all PTA events, two people must count all money, fill out and sign these two forms verifying the funds received, then deliver the money and the receipts to the treasurer. If you have any questions, please contact Jennifer Kenna.
